Wake Forest University offers its final free flu shot clinic for faculty, staff, and retirees on October 19 at Graylyn International Conference Center.

Advanced registration is required. The deadline to register is 2:00 p.m. October 18. 

Spouses and dependent children (8 years and older), who are enrolled in the university’s BlueCross BlueShield medical plan are also eligible. Speak with your primary care physician to see if the flu shot is right for you. All supplies are preservative and latex-free.

Important reminders:

Safety: Individuals are required to wear a mask

Identification: Faculty, staff, and retirees should bring their Deacon OneCard. Spouses and dependent children should bring their BCBS insurance card.

Attire & Seating: Wear clothing that allows easy access to the upper arm. Short sleeves are preferred

Documentation: Bring a completed Flu Shot Consent Form (download in English or Spanish).

Note: Wake Forest University is requiring all faculty, staff, and students to receive a flu shot to help to reduce the burden on resources of the healthcare systems on and off campus while also promoting the health and wellbeing of the campus community.
