Wake Forest is offering two separate wellbeing incentives, $50 and $150, for Reynolda Campus medical plan participants for the new plan year. These incentives are offered through the BlueCross BlueShield of North Carolina Healthy Outcomes program. The program will begin July 1, 2015 and run through June 30, 2016. Please note that the $50 incentive is not a prerequisite to earn the $150 incentive. 2. Click on the “Wellness” tab

3. Complete the Health Assessment between July 1 – December 31, 2015

4. Earn $50 credit towards your medical plan premiums January 2016

5. No credits for the Health Assessment or any other wellbeing activity will be awarded on the BCBSNC website during this incentive period

Steps to earn the $150 wellbeing incentive: During this incentive period, the requirement is to earn 80 credits by completing wellbeing activities. 1. Log in at www.BlueConnectNC.com 2. Click on the “Wellness” tab 3. Complete 80 credits between January 1 – June 30, 2016 4. Track your progress at www.BlueConnectNC.com 5. Earn $150 credit toward your medical plan premiums July 2016
